A copy of the abstract or notice of support judgement and an information release form signed by the debtor will need to be uploaded with the request.​

A written demand will be provided within 15 ​days. Please note, verbal quotes cannot be provided.

Please make checks payable to “California State Disbursement Unit”. Include the participant’s full name, participant number, and “LIEN” on the check for accurate identification and credit.
